When booking your appointment please supply:

• A GP referral letter.  

• We can see patients with simple, urgent problems (such as a new rash in a healthy person, or a new growing lump on the skin) without a referral letter.  In this case, please supply the following information:

  • Your full name (including your title - Mr, Mrs, Miss etc), date of birth, home address, home and mobile phone number, email address, and your GP's name and address.  

  • Preferred means of communication (email versus post - please do check your junk email box for our response, as some emails might automatically be diverted away from your inbox; you will normally receive a response within 24 hours during the working week). If you have any doubt, please telephone 07624 361254 and leave your telephone number. 

  • The nature of the problem for which the appointment is being made (this helps us to ensure that the appointment is appropriate to your needs).

  • Please advise us if there are any special factors of which we should be aware, including cognitive impairment or deafness in later life. In this case, a GP referral letter is very strongly preferable.

  • GP referral is essential for patients with complex medical problems, including those who have seen other doctors with the same problem, those on multiple medicines or with multiple other health problems.

  • If the problem is a complex wound, affects the genitalia, is a complex medical problem or is very distressing, we can advise as to the most appropriate consultation.  Occasionally a one-hour initial consultation or a home visit is most appropriate.  These cost £400.
